Restoration to the Community Services
The purpose of restoration to the community services is to assist clients to reintegrate into their community activities, social interactions and hobbies and also to help them re-establish a positive daily routine.
The main focus is on promoting independence with meaningful activities at home and access to the community. Opportunities for volunteer work or employment transition can also be explored if relevant to the individual’s goals. A detailed initial assessment is completed to identify barriers, set goals, establish a plan and recommend strategies to implement the plan. Regular updates are provided to the referring agent to provide information about the client’s progress.
Individualised goals are set together with the client, and are based on their personal areas of interest. Generally these goals are focused towards enhancing participation in community activities, social interaction, pursuing hobbies, joining groups of people with similar interests and/or increasing independence around the home. Once the goals are clear, a gradual plan to implement these goals is developed with the client to ensure they can achieve their intended outcomes in a safe and realistic way.
The Progressive Goal Attainment Program (PGAP) may be completed as part of this service.
A referral for Restoration to the Community Services can be made:

- when the client has complex injuries or conditions;
- when the client is not likely to return to work in the foreseeable future.
