What to Expect at Your First Appointment
Your first appointment will generally involve you coming out to visit with one of our staff members at one of our offices. It can be helpful to bring your most recent medical certificate so we know what your current capacity is.
If you have been referred for vocational counselling or are working towards new employment you may also be asked to bring a copy of your resume and your qualifications. During the first meeting you may be asked to complete some questionnaires as well as answering some questions to help us establish the best way we can support you.
You will have an opportunity to discuss your goals. We try to keep these meetings as informal as possible, so you feel comfortable and relaxed.
You are welcome to bring a support person if you would like.

Authority to Obtain Information
This form is your opportunity to provide us with consent to discuss your situation with other people involved (e.g. your doctor, physio or employer) to assist you in achieving your goals. It is your choice who you allow us to speak with and you can change this authority form at any time.
